DEMOGRAPHICS

AGE: 30 Years                      
RESIDENCE: Matuga

PARITY: Para 1+0                      
GRAVIDITY: Gravida 2

LMP: 01/01/2017

EDD: 08/10/2017

GBD: 32 Weeks

CHIEF COMPLAIN

Per vaginal bleeding -1 day

HISTORY OF PRESENTING ILLNESS (HPI)

The patient was well until yesterday 9pm while making supper when she noticed that she was bleeding vaginally. She reports that the bleeding was moderate in amount, bright red in color and foul smelling. The patient has no history of recent trauma and denies having any abdominal pain, no per vaginal discharge and no drainage of liquor. There is no other site that is bleeding nor does she have any known bleeding disorder. However, she was managed for a threatened miscarriage when she was 10 weeks pregnant. Up to now, she has changed sanitary pads 4 times which are not heavily soaked and has not taken any medication. She is perceiving normal fetal movements.
